Crashes : Access_violation

I've seen that I'm not the only one with crashes to the desktop on EVERY loading scene! i still don't get it - this is the error:

can anyone help me? I've already formated my hd but same thing...P4 2.4 ghz, 768mb ram, GeForce 4 ti 4600: should work!!!!
