How to complete all 49 faction missions and get acheivement ?

I have completed the all of the main story and all the faction missions i can seem to find but i seem to only have do 45? does anyone know what to do ?
Im really confused

I just want to get the acheivement and complete all 49 faction missions

Has this happened to anyone else or am i on my own ?
