what is the extent to what a patch can change on a game? can it be completely changed with graphics and such?

i try to explain what a patch is : a patch is a small installer that fixes or repairs bugs in the game such as computerproblems, graphiccardproblems, sound problems ,etc.
i hope it's clear now what a patch is.
