Bug with Smooth Operator (potential spoilers)

On a 360, FYI.

I have tried this multiple times, and am still not getting smooth operator when trying to reach Darrow in Panchea. This is the first Panchea objective, when you enter the facility. When the conversation with Darrow starts, I do get the Ghost bonus, but no Smooth Operator. No alarms, not seen by turrets, no failed hacks because I'm not hacking anything.

Anyone else have this issue or any suggestions?

So after all that, I decided to play through to the end and got the achievement anyway. Prior to this run, I got Pacifist and Legend, but no Foxiest. On this play through, which was no different from the previous one, I got Foxiest. Odd. Can you not get all 3 on the same run-through?

Not all objectives offer the Smooth Operator bonus.

And yes you can get all three on one play through, seeing how I did :).
